CONTRAINDICATIONS: Hypersensitivity to any of the ingredients. During pregnancy and lactation. In patients with obstructive disease of the bile ducts and severe hepatic failure.
CAUTIONS:
Renovascular hypertension: Risk of severe hypotension and renal insufficiency when patients with bilateral stenosis of renal artery stenosis or artery of a functioning kidney are treated with drugs affecting the renin-angiotensin-aldosterone system.
Renal failure and kidney transplantation: MICARDIS * When given to patients with renal insufficiency is recommended continued monitoring of plasma potassium and creatinine. There is no experience with the drug's use in patients with recent kidney transplant.
Intravascular volume depletion, can occur manifestations symptomatic hypotension, especially after the first dose administration in patients with volume depletion and / or sodium after extensive treatment with diuretics in salt-restricted diets, diarrhea or vomiting. These conditions, especially volume depletion and / or sodium should be corrected prior to administration of MICARDIS *.
Other conditions with stimulation of the renin-angiotensin-aldosterone system: In patients whose vascular tone and renal function depend predominantly on the activity of the renin-angiotensin-aldosterone system (eg patients with severe congestive heart failure or underlying renal disease, including renal artery stenosis), treatment with drugs that affect this system has been associated with acute hypotension, azotemia, oliguria, and rarely with acute renal failure.
Primary aldosteronism: patients with primary aldosteronism generally will not respond to antihypertensive treatments that act by inhibiting the renin-angiotensin system, so this medication is not recommended in these cases.
Aortic and mitral stenosis, hypertrophic obstructive cardiomyopathy: As with other vasodilators, special caution is recommended in patients with these diseases.
Hyperkalemia: Hyperkalemia may occur during treatment with drugs that affect the renin-angiotensin-aldosterone system, especially in cases of renal failure and / or heart disease. It is advisable to monitor serum potassium in patients at risk.
Based on experience with drugs affecting the renin-angiotensin-aldosterone system, concomitant use of potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium or other drugs that increase potassium levels (heparin, etc.) can lead to an increase in serum potassium levels, so caution should be exercised when administered simultaneously with MICARDIS *.
Hepatic impairment: The drug is eliminated with the bile. In patients with obstructive disease of the bile ducts or liver failure is expected to have decreased their disposal. MICARDIS * should be used with caution in these patients.
Sorbitol: A daily dose of 40 mg of MICARDIS *, which contains 169 mg of sorbitol, or 80 mg to 338 mg, are not suitable for patients with hereditary fructose intolerance.
Other: As with other ACE inhibitors, angiotensin receptor blockers, including MICARDIS *, are apparently less effective in lowering blood pressure in black hypertensive patients, probably due to the high prevalence of stages with concentrations low renin.
As with other antihypertensive therapy, excessive lowering of blood pressure in patients with ischemic heart disease can result in a myocardial infarction or cerebral vascular disease (CVD).
There have been no studies on the ability to manage vehicles and equipment, but must take into account that dizziness or drowsiness may occur as a result of antihypertensive therapy.
RESTRICTIONS OF USE DURING PREGNANCY AND LACTATION: MICARDIS * should not be administered during pregnancy and lactation. We should make a change to a suitable alternative if you are planning a pregnancy.
During pregnancy, substances that act directly on the renin-angiotensin system can cause injury and even death to product development, therefore, telmisartan is contraindicated during pregnancy.
If it is determined during medication pregnancy with telmisartan, the latter should be discontinued immediately.
The recommended dose is 40 to 80 mg once a day. Some patients may benefit from a dose of 20 mg. In cases where failure to obtain the expected decline in blood pressure, may be increased to 80 mg a day.
When considering an increase in dosage should be noted that the maximum antihypertensive effect is usually obtained after 4 to 8 weeks after starting treatment.
In patients with severe hypertension have used dosages higher than 160 mg alone or in combination with 12.5 to 25 mg of hydrochlorothiazide daily, they have been effective and welltolerated.
No dose adjustment is necessary in patients with renal impairment, including hemodialysis. The drug is not withdrawn from circulation by hemofiltration. In patients with hepatic impairment or moderate, the maximum recommended daily dose is 40 mg. In elderly subjectsis not necessary dosage adjustments.
No information on the safety and efficacy of MICARDIS * in children and adolescents.
MICARDIS * can be administered before or after food.
